Supporting Documents

Note:   

  • All documents submitted will not be returned.

  • Application will only be processed after all required documents have been submitted and application fee has been paid.

1) Transcript of University degree

  • If you are a CUHK graduate, please provide a copy of transcript.

  • For those who are not CUHK graduates, they are required to apply for an official transcript from their former/current university, and send it to the SMHS Programme Office. The transcript must be sent by the University to the SMHS Programme Office directly

    • If the University issues hardcopy: To be considered “official” your transcript must remain sealed. Applicants must not open the envelope as this will void the transcript.

    • If the University issues e-transcript: Please request to send it to smhs@cuhk.edu.hk directly.

Submission Methods:

  • CUHK Graduates:  Online submission to Application System

  • Non-CUHK graduates: Request the former/current University to submit one to the Programme Office

2) Copy of University certificate of graduation

3) Copy of resident Identity Card

4) A written statement explains the purpose of application and motivation (one to two pages).

5) Copy of certificates of professional qualifications

  • Examples: certificates of registration from Medical Council of Hong Kong / Physiotherapists Board of Hong Kong / Nursing Council of Hong Kong / Occupational Therapists Board of Hong Kong / Hong Kong Society of Certified Prosthetist-Orthotists; certificate of registration as a Teacher issued by Education Bureau of HKSARG

6) Two confidential recommendations (either in hardcopy OR softcopy)

  • Softcopy:  Applicants can input information of the TWO referees through the Application System. The University will send invitations for confidential recommendations to the referees.

  • Hardcopy:  Forms can be obtained at the Application System.

  • Send the documents to Programme Office by post

  • Item 4:  Please upload to the "Certificate" file category. 

Additional Documents Required

i) For applicants who did not graduate from the universities in Hong Kong:

  • If applicants are graduated from non-English speaking countries (e.g. China, Japan), they are required to provide TOEFL or IELTS score report for assessment of English Language Proficiency. Applicants should apply for an official score report from the test institute, and send it to Programme Office.

ii) Documents that are not in English or Chinese must be accompanied by an official English translation.

iii) For applicants who did not study human anatomy and human physiology courses during their undergraduate / postgraduate studies:

  • Applicants have to submit the e-certificates as a proof of fulfillment.

iv)  Applicants who obtained degrees from universities in Mainland China are required to provide a valid Online Verification Report of Higher Education Qualification Certificate (教育部學歷證書電子註冊備案表) and Online Verification Report of Higher Education Degree Certificate (中國高等教育學位在線驗證報告) issued by the CHESICC (學信網) together with their degree and graduation certificate.
