APPLICATION PROCEDURES 報名程序
To secure your application, please complete the following procedures by the application deadline:
a.
Complete the Application Form.
Applications open on 15-Dec 2024
b.
Upload all the following supporting documents to the application form:

Copy of HKID card
-
Copy of a valid Basic Life Support (BLS) certificate, or equivalent;*

Copy of certificate of Level 2 Sports Trainer issued by The Chinese University of Hong Kong or Sports Medicine Australia.

A letter from an employing club or organization confirming a minimum of 50 hours of experience over the previous 3 years.^
c.
Submit course fee# by post to the course administration office.
*For details of BLS certification requirements, please refer to “Course Information” ➜ “Graduation Requirements”.
We accept certifications issued by the following local organizations:
Hong Kong St. John Ambulance 香港聖約翰救護機構
Hong Kong Red Cross 香港紅十字會
The Auxiliary Medical Service 醫療輔助隊
Hong Kong Society of Professional Medical Care 香港專業護理學會
e.g. BLS certificate or AED & CPR & First Aid certificate
^The letter MUST be provided when applying for the course.
#Payment Details:
Course Fee MUST be paid by CROSSED CHEQUE payable to "The Chinese University of Hong Kong".

To secure your application, please mail the cheque to the course administration office.
