To secure your application, please complete the following procedures by the early-bird application deadline or final application deadline.

Course Fee MUST be paid by CROSSED CHEQUE payable to "The Chinese University of Hong Kong".

Upload all the following supporting documents to the application form:

Copy of HKID card

Copy of a valid Basic Life Support (BLS) certificate, or equivalent;*

Copies of Certificate of Graduation;

Copies of Professional Qualification (if any).

Submit course fee# by post to the course administration office.
